SOCIAL SERVICES APPLICATIONS
Applying for social services does not have to be an overwhelming experience. We help older adults, people with disabilities, and their families navigate the process of accessing care coordination and related services, so you can spend less time dealing with paperwork and more time caring for your loved ones.

My passion has always been helping people navigate life's most challenging transitions with dignity, compassion, and confidence.
As a Master Social Worker in New York State, I am committed to empowering individuals and families by providing personalized guidance, advocacy, and practical solutions when they need them most.
I earned my bachelor's degree from Mercy University and my Master of Social Work from New York University. Throughout my career in public service, I have worked with individuals facing complex personal, medical, and social challenges, helping them understand complicated systems, overcome barriers, and connect with the resources they need to move forward.
My practice serves two primary populations:

Older adults, individuals with disabilities, and their families as they navigate Medicare, Medicaid, long-term care planning, nursing home placement, hospital and rehabilitation discharges, home care services, durable medical equipment, and other complex healthcare and social service systems.

Individuals returning to the community following incarceration by helping them navigate the steps toward rebuilding their lives, identifying employment resources, and connecting with essential community services.

Our mission is to empower individuals and families through compassionate based aftercare that promotes stability, independence, wellness and lasting success. We will help connect individuals and their families to resources, support systems and opportunities they need to thrive.

Our goal is to strengthen communities by empowering vulnerable populations access the support, resources and relationships necessary to maintain stability and to reach their full potential.
